I strive to create, an emotionally and physically safe, and positive environment for both students, and myself. I agree to conduct myself in a kind, and respectful manner, always. I agree to show up on time for every class. I agree to provide materials, class content, and instruction, to the best of my ability. I take pride in my work ethic and giving my all to every student. If I must cancel a class (I will do my utmost not to), due to an emergency, I will give as much notice as possible and a refund will be issued for that class. – Robin Schwartzburd

I, _________________________________, agree to pick up my minor participant on time. If I fail to do so, I authorize Robin’s Nest Art School, to charge me a late pick up fee of $5 for every 5 minutes that I am late.
I agree to register and pay for a class at least 12 hours in advance, or I will not show up, unless an agreement has otherwise been made.
I agree that a cancellation, with less than 24 hours of notice, prior to the class start date, will not be refunded. Once a class session has begun, no refunds will be given if a student is absent.
I understand, a class session may be cancelled, due to lack of enrollment, within 48 hours of scheduled start time, and a full refund will be issued.
I agree, as a parent or guardian, if staying to observe the class, I will not disrupt the class or the instructor.
Participants, guests, and observers agree to conduct themselves in a kind and considerate manner, to both the instructor, other guests, and the students. Negative comments regarding artwork will not be tolerated.
Robin’s Nest Art School reserves the right to refuse service to any Participant for any reason.
I agree to the above terms and conditions, if a participant or parent/guardian of a participant, violates any of these terms or conditions, Robin’s Nest Art School may disenroll the participant from the program, without refund or compensation.

Medical Release. I, the undersigned parent/legal guardian of the undersigned minor participant (“Participant”), or adult student (“Participant”), authorize Robin’s Nest Art School staff to take whatever actions may be necessary to obtain emergency medical care if warranted. These actions may include, but not be limited to, the following: Attempting to contact parent or legal guardian, attempting to contact parent or legal guardian through any of the persons listed above, calling 911 for assistance and have Participant transported by ambulance to a hospital if recommended by emergency personnel.
I, the parent/ legal guardian of Participant, hereby further authorize any physician or medical staff of licensed hospital to provide treatment as is necessary until I can be notified. I understand this authorization is given in advance of any treatment being required.
Release, Hold Harmless and Assumption of Risk. Participant desires to participate in the class activities described herein and other activities related thereto (the “Activities”). Participant, and his or her parent or legal guardian, if participant is under 18 years old, recognizes that the Activities are hazardous and understand the risks and dangers associated with participation in the Activities, including but not limited to, communicable diseases, illnesses, viruses, bodily injury, disability, paralysis and death. The undersigned accept and assume such risks and responsibilities however caused or alleged to be caused by any party with the exception of those risks caused by gross negligence, fraud, willful misconduct or violation of law. In consideration for Participant being permitted to participate in the Activities, each of the undersigned hereby waives, releases, discharges, indemnifies and holds harmless Robin’s Nest Art School and Robin Schwartzburd, its, employees, agents and any other related parties, and Valley Oak Charter School from any and all claims for damages for wrongful death, personal injury, or property damage arising out of Participant’s participation in the Activities and caused by negligence (passive or active), strict liability or otherwise (except for such injury, wrongful death or property damage caused by Robin,s Nest Art School,s gross negligence, fraud, willful misconduct or violation of law). This release is intended to be binding on each of the undersigned’s heirs, beneficiaries, personal representatives, next of kin, spouse and assigns. I/We have read the above waiver, have been fully and completely advised of the potential danger incidental to engaging in the Activities, are fully aware of the legal consequences of signing this waiver and have signed it voluntarily.
Activity Refund Policy. Absolutely no refunds will be granted after the class begins. If a class is cancelled by Robin’s Nest Art School, a refund will be processed. A credit voucher can be issued for the full amount and is valid for 1 year. Activities that do not reach the minimum enrollment requirement may be cancelled and a refund and or credit will be issued. There is a $28.50 returned check fee.
